Qlarant provided External Quality Reviews of services and supports provided to individuals with intellectual or developmental disabilities. Qlarant performed Quality Service Reviews (QSR) to evaluate the following at the individual, program, community services board (CSB), regional (health planning region), and statewide levels:
ā¢ whether individualsā needs are being identified and met through person-centered thinking and planning (including building on the individualsā strengths, preferences, and goals);
ā¢ whether services are being provided in the most integrated setting appropriate to individualsā needs and consistent with their informed choice; and
ā¢ whether individuals are having opportunities for integration in all aspects of their lives (e.g., living arrangements, work and other day activities, access to community services and activities, and opportunities for relationships with non-paid individuals).
Qlarant conducted Person-Centered Reviews (PCRs) of a statistically significant sample of individuals receiving services and supports, using PCR results to develop recommendations to improve (a) person-centered thinking and planning; (b) provision of services and supports in the most integrated setting appropriate to individualsā needs and consistent with their informed choice; and (c) opportunities for integration in all aspects of individualsā lives.
Qlarant conducted Provider Quality Reviews (PQRs) of direct services and supports providers serving individuals selected to participate in PCRs. Used PQR results to develop and present recommendations for improving practice and service quality to the provider.
Qlarant completed QSR Assessments at the CSB, regional, and statewide levels that included PCR and PQR results, assessment of services system key performance indicators, individual outcomes, critical incidents, injuries, and mortality data; National Core Indicator (NCI) individual and family/others in individualsā lives survey results; services system compliance with contractual/regulatory/Settlement Agreement requirements; and other data, as agreed upon by Qlarant and the Department of Behavioral Health & Developmental Services.
Qlarant submitted QSR Assessment Reports to the DBHDS, including recommendations to DBHDS, State Quality Improvement Committee, and Regional Quality Councils for improving service quality and practices at the provider, CSB, regional, and statewide levels.
