Eastern Shore Wellness Solutions is a community-based, health equity advocate created to influence change and advance racial equity for all marginalized and underserved populations on Maryland’s Eastern Shore through awareness, education and outreach. Qlarant Foundation supports the Community Health Outreach Workers Program to improve health care delivery to the disadvantaged populations in Dorchester and Caroline counties.
The Community Health Outreach Workers Program focuses on advancing health equity, reducing racial and health disparities among marginalized populations, improving quality healthcare access, and improving health outcomes in underserved areas. The program is operated by non-clinical trusted members of the community who have personal experiences with chronic disease and care management. The program provides care coordination for English and Spanish speaking participants and their primary care providers. The focus of the program is on families impacted by chronic conditions, including diabetes, hypertension and asthma.
