- Qlarant was contracted to conduct a Root Cause Analysis and develop an Action Plan that met all Centers for Medicare & Medicaid Services (CMS) requirements related to the subject care center (facility) to assist in carrying out the actions described in the Systems Improvement Agreement between CMS and facility.
- Qlarant engaged highly expert individuals to conduct an onsite Root cause Analysis at facility. Facility delivered a report of the Root Cause Analysis findings.
- Using the findings from the Root Cause Analysis, Qlarant developed an Action Plan and submitted a draft to facility officials for approval by CMS. The Action Plan includes a list of evidence-based recommendations for changes and Improvements necessary for facility to achieve compliance with Federal Conditions of Participation (CoPs).
- Qlarant monitored implementation of the Action Plan at the facility, to provide monthly reporting to the Centers for Medicare & Medicaid Services (CMS), and to provide Technical Assistance.
- Qlarant maintained an Intermediate Care Facility/Individuals with Intellectual Disabilities (ICF/110) expert onsite as needed following the acceptance of the Action Plan. The Action Plan resulted in a tool Qlarant would use to monitor progress of the facility in ameliorating outstanding issues relative to the Federal Conditions of Participation (CoP) compliance.
- Qlarant provided Technical Assistance/Training including:
- Person-Centered Training
- Client Protections and possible Mock survey consultation
- Governing Body
- Behavior Supports consultation
- Qlarant provided a monthly written report to CMS, and simultaneously sent copies of the reports to the State Survey Agency and facility. The reports will included at a minimum the following information:
- Dates and times of the visits by the external expert(s),
- Summary of observations made during the visits,
- Summary of any Interviews conducted and with whom,
- Summary of any records reviewed,
- Any quality concerns Identified,
- Any complaints received by the Monitor,
- Assessment of facility in meeting established goals outlined in the Action Plan, and
- Obstacles on each Item of facility’s Action Plan and proposed solutions to those barriers.
Qlarant provided monitoring services, trained staff to conduct person centered interviews; worked with facility staff to rework person centered plans to bring them into compliance with federal regulations; trained staff to implement the plans; provided consultative nursing services and consultative mentoring of new residential director.
